• Your email address for the required online intake form

We’ll text you back to fine-tune an appointment time that works well for both of our schedules. Your intake form must be completed online before your appointment.

You may also book directly online. If you do not see a date or time that works for you soon enough, add yourself to our waitlist and we’ll contact you if an earlier opening becomes available.
